About the Service
The Positive Futures Leaving Care Service is a dedicated team focusing on children aged 16 and over who are approaching the end of their care journey. We support their transition to adulthood through pathway planning, independence building, and continuous safeguarding support.
Your next role
As a senior social worker you will provide direct, hands‑on support to former pupils of the Ofsted rated and Social Work England registered practice. Your key responsibilities include:
* Supporting children in late teenage years to achieve education, employment and personal development goals.
* Building trusting relationships with children, carers, families and multi‑agency professionals.
* Delivering safeguarding assessments and practice interventions in line with national standards.
* Planning and reviewing service provision to reduce barriers to successful adulthood.
* Collaborating with local agencies, schools, community groups and health services to co‑ordinate care.
* Traveling locally and occasionally outside of Swindon, including evening visits, to complete assessments and reviews.
About You
You will be aspirational, motivated and committed to improving outcomes for children. You will bring:
* A recognised Social Work qualification and registration with Social Work England.
* Comprehensive post‑qualifying experience, working with Cared for Children or Care Experienced Young People.
* Solid safeguarding knowledge and proven practice skills.
* Excellent communication and empathy with a track record of building trusting relationships.
* Strong organisational and time‑management skills to meet flexible working patterns.
* Ability to travel as part of service delivery, including evenings.
Additional requirements:
* Full UK driving licence.
* Fluent in English (as required by Part 7 of the Immigration Act).
