Community Children's Staff Nurse

Providing high quality, holistic, family‑centred nursing care for children in the community setting who meet the criteria for referral. The post holder will be an effective member of the Community Children’s Nursing team, providing skilled nursing care in partnership with organisations and child patients, carers and families. The post holder will work independently, with supervision, within the community in line with local and national policies. The post may also support the special schools and involve working across sites during a normal working week.
Main Duties
* Carry out clinical visits to family homes and educational settings providing care that prevents hospital admissions, advise families and allied health professionals, and maintain good clinical skills.
* Manage a caseload of patients while being reactive to day‑to‑day requests from all patients requiring intervention that day.
* Communicate and work as part of a busy team to provide good quality care in the community.
* Partner with families and the multi‑disciplinary team to provide a high standard of care in appropriate settings to maximise life opportunities for the child and young person.
* Demonstrate knowledge, skills, competency and innovative practice in paediatric nursing care.
* Continuously evaluate clinical practice and recommend changes.
* Provide and maintain a high standard of skilled nursing care/skills for children with additional health needs, including taking bloods, re‑passing nasogastric tubes and gastrostomies, recording oxygen saturations, providing and giving rescue medication, suction, tracheostomy care, injections, dressings, pressure area care and assessments.
* Assist in the completion of detailed holistic assessments of children and families with consideration to cultural differences.
* Clinically assess children in homes/schools as appropriate and provide support, information and the necessary advice or care to maintain safety of the child, including escalating and calling ambulances if required.
* Assist in the completion of Education and Health Care Plans.
* Establish or maintain relationships with the child and family when relevant whilst the child is an inpatient to help facilitate a smooth transition.
* Provide training and support to school and nursery staff and other community areas.
* Record accurately and timely the involvement with the child and family and report any changes affecting them appropriately.
* Work with other agencies to ensure appropriate transition of care in a timely fashion to meet ongoing needs of the child/young person and family.
* Develop and maintain effective communication networks with professionals from other agencies and voluntary groups.
* Support the development of integrated care pathways.
* Take referrals as appropriate within the agreed scope of the role and refer children and families to other specialists and generic services as required.
* Be accessible and available to key stakeholders to ensure advice is obtained in a timely manner.
* Provide cover for other nurses in the team as required, depending on scope of practice.
* Maintain knowledge and competence in computer skills to access drives, databases, email and approved internet resources and critically appraise sources of information and their evidence base.
* Maintain clinical skills and practice to support and guide clinical teams and other multi‑disciplinary teams.
* Provide training for parents/carers/schools/nurseries about complex health needs and support services available, in conjunction with band 6 members of the team.
