At Mountain Community Health, our triage nurses are often the first voice patients hear and the steady hand that helps them take the next right step. We’re looking for a nurse (RN or LPN) who is ready to bring both clinical skill and compassion to this vital role.
What You’ll Do
* Assess patient needs by phone and in-person, using sound clinical judgment.
* Direct patients to the right care—from same-day visits to self-care guidance.
* Collaborate with providers to ensure seamless, safe, and timely care.
* Be a trusted resource for patients, offering not just answers but reassurance.

Requirements
What We’re Looking For
* current RN or LPN license (State of Vermont in good standing)
* Strong communication and listening skills.
* The ability to think quickly, prioritize, and stay calm under pressure.
* A genuine desire to support patients and their families
* Min 5 years ambulatory outpatient experience
