Job Summary
Provide specialist Occupational Therapy assessment, treatment and review of patients with advanced palliative or complex deteriorating neurological conditions. Work as a Band 6 Occupational Therapist within the Community Specialist Palliative Care Team, supporting patients and families with rapidly changing health‑care needs in their place of residence.
* Deliver a comprehensive range of evidence‑based assessment and treatment to adults in Coventry with specialist palliative needs in the patient’s own home or other community setting.
* Follow defined referral criteria to the Specialist Palliative Care Team.
* Liaise closely with the interdisciplinary team (Occupational Therapists, Physiotherapists, Therapy Technicians) and other Community services.
* Provide advice and guidance to patients, carers and staff working in health and social care.
* Support professional development of staff within other services where appropriate.
* Contribute to service development, meeting key performance indicators and activity monitoring.
* Adhere to the lone worker policy.
Core Responsibilities
* Deliver evidence‑based assessments and interventions to patients referred to the service.
* Safely manage a palliative care and neurological caseload, addressing physical, psychological, behavioural, perceptual, cognitive, sensory and environmental needs in an unstable or deteriorating phase.
* Adopt a patient‑centred, sensitive approach, collaborating with patients (and/or carers) throughout assessment, planning, intervention, evaluation and reporting.
* Maintain high knowledge of neurological, orthopaedic, musculoskeletal, care‑of‑the‑elderly, respiratory and fall‑related conditions, understanding the impact of multi‑pathological rapid presentation.
* Participate in interdisciplinary and multidisciplinary meetings; facilitate effective caseload management.
* Support quality and activity monitoring systems and evaluate the effectiveness of the service.
Additional Requirements
* Provide an immunisation and vaccination report from your UK occupational health provider or GP.
* A full UK driving licence is essential; an international licence is not accepted.
