Review Community Support Officer - Older People

About the role:
The Review Community Support Officer (Grade 7) role within Adult Services focuses on carrying out person-centred assessments, reviews and care and support planning for older people, using a strengths-based and outcome-focused approach. The post works across community and hospital settings to promote independence, wellbeing and social inclusion, in line with the Social Services and Well-being (Wales) Act 2014, the Mental Capacity Act 2005 and safeguarding legislation
About you:
Strong communication and report-writing skills, working effectively with
older people, carers and professionals
Ability to assess, review and manage risk, using a person-centred,
strengths-based and outcome-focused approach
Proven skills in prioritising workload and managing a caseload, including
complex and challenging cases
Effective problem-solving, negotiation and decision-making skills within
limited resources
Experience of multi-disciplinary and partnership working, including
attending and chairing meetings
Confident IT and case-recording skills, with a clear commitment to
safeguarding, confidentiality and equality
What you will do:
Carry out person-centred assessments, reviews and re-assessments for
older people using a strengths-based, outcome-focused approach
Develop, review and commission care and support plans, including
long-term care, promoting independence, choice and wellbeing Assess and manage risk, including undertaking Mental Capacity Act assessments and
Best Interest decisions where required
Work collaboratively with Reablement, health and partner agencies,
including chairing and attending multi-disciplinary meetings
Identify, report and escalate safeguarding concerns and unmet need in
line with policies and legislation Prioritise and manage a caseload,
participate in duty rotas, and seek supervision appropriately
Maintain accurate, timely electronic case records and ensure statutory
documentation is completed and shared correctly
