Overview
Job summary at Northamptonshire Healthcare NHS Foundation Trust. We are looking for a Health Care Assistant to join our team. The service aims to provide an excellent standard of person-centred planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. The successful candidate will deliver safe, effective care for the patient experience. You will work closely with the multi-disciplinary team to support the service and focus on delivering quality outcomes and improving the patient experience. You should be enthusiastic, innovative and highly motivated, with the ability to be flexible to the needs of the service and deliver a high standard of care in a supportive and forward-thinking environment. The service operates from 8am to 6pm, seven days a week, over a shift pattern. Weekend and bank holidays are a requirement. A car owner/driver with a current driving licence is essential or access to a car (unless you have a disability as defined by the Equality Act 2010).
Main duties
As a Healthcare Assistant, you will:
* deliver safe, effective care for the patient experience
* support patients to remain in their home environment
* be professionally accountable for the delivery of care
* utilise technology to update records as you go
Your patients could be of any age, often elderly, and may include recent hospital discharges, terminally ill individuals or those with physical disabilities.
The service aims to provide an excellent standard of care and works as part of a multi-disciplinary team in Northamptonshire to support the service, focusing on delivering quality outcomes and improving the patient experience.
