Responsibilities
* Serve as a key point of contact and support for young people leaving care.
* Coordinate support and resources from various agencies to ensure smooth transitions into adulthood.
* Provide direct work and targeted interventions for young people aged 16 to 25, helping them secure education, employment, and stable housing.
* Engage with young people, especially those harder to reach, fostering connections and enhancing life outcomes.
* Support and challenge families and agencies to drive improvement in outcomes for care leavers.
* Deliver a duty service to provide crisis support and ensure ongoing advice and guidance are available.
