This role involves working as a member of the Family First Service, delivering targeted interventions to families with children aged 0-18 years, addressing complex needs such as parenting skills, relationship issues, emotional health, and more. The aim is to support families in achieving better outcomes, including improved school attendance, reduced risk-taking behaviors, and enhanced employability.
Family First Case Managers will adopt a multidisciplinary approach, utilizing their knowledge of available services and interventions. They will undertake holistic assessments, develop outcome-focused family plans, and coordinate their delivery. This includes delivering practical support, coordinating multi-agency processes, and delivering evidence-based programmes such as parenting and child development initiatives, considering a Contextual Safeguarding approach.
Case Managers will also help develop and deliver prevention and early intervention programmes, participate in practice improvement activities, and may lead thematic projects.
Duties and Responsibilities

1. Engage assertively with families to ensure participation in available support, especially for families with complex needs.
2. Undertake early help assessments, identify risks, and develop family plans addressing these risks, ensuring the child's voice is heard throughout.
3. Act as the lead professional, coordinating service delivery and acting as the main contact for the family, collaborating with statutory services as needed.
4. Lead and coordinate the Team Around the Family process, conducting reviews to monitor progress.
5. Promote approaches such as 'Think Family' and 'Signs of Wellbeing', using restorative practices to support family strengths and needs.
6. Maintain accurate records on the Early Help Module, support audits, and update service information systems.
7. Provide parenting support through formal and informal programmes, including in-home support where appropriate.
