Main Responsibilities
Deliver high-quality social work services to adults and families, focusing particularly on complex cases.
Conduct comprehensive needs assessments and create tailored care and support packages.
Balance risk and autonomy to promote safe and independent living for service users.
Provide expert advice and support to colleagues, students, and other professionals.
Promote independence and informed decision-making through collaboration with individuals, families, and communities.
Design and commission cost-effective care solutions.
Engage in preventative interventions using tools such as reablement and telehealth.
Maintain accurate and timely records on both electronic and paper systems.
Present cases to the Resource Allocation Panel and contribute to legal proceedings where necessary.
Build positive working relationships with service users, carers, and partner agencies to gather information and resolve conflict.
Stay up to date with current legislation, policy, and best practices within adult social care.
Essential Skills and Knowledge
In-depth understanding of the Making Safeguarding Personal approach.
Solid working knowledge of the Mental Capacity Act 2005, Human Rights Act 1998, and Care Act 2014.
Strong professional ethics and values focused on empowering individuals and achieving positive outcomes.
Ability to work preventatively and collaboratively with families, carers, and the wider community.
Experience in creating efficient, value-for-money care plans and liaising effectively with multidisciplinary teams.
