The Digital Communications and Operations Coordinator will play a pivotal role in supporting the ongoing development and operational effectiveness of Moreton & Meols and Wallasey Wellbeing PCN.

The postholder will lead on digital communications, support the PCN's digital transformation agenda, provide performance reporting and administrative support, and coordinate the operational delivery of Enhanced Access services. The role requires an organised, proactive individual who can manage multiple priorities, communicate effectively with a range of stakeholders, and contribute to the efficient running of PCN services.


Main duties of the job

The Digital Operations Coordinator will provide administrative, communication and data coordination support across the PCN.

The role will include, supporting day-to-day administrative processes, monitoring PCN inbox and correspondence tasks and managing patient contact.

A key part to the role will be using clinical systems such as EMIS and other local reporting tools to gather data to help identify patients who may benefit from PCN Services and also support practices with QOF (Quality and Outcomes Framework) and IIF (In pact and Investment Fund).

The postholder will also be responsible for operationally coordinating the PCN Enhanced Access Service, booking clinicians to cover sessions and ensuring the service runs smoothly.

The role play's a key part in the PCN's Digital Transformation and communications, managing social media accounts, website and monthly bulletins.

They will work closely with administrators, clinicians and managers, keeping the services organised, information accurate, communications up to date and patients connected with the right support.

Moreton & Meols and Wallasey Wellbeing Primary Care Networks (PCNs) are a group of 9 GP Practices working together with other healthcare organisations to improve the health of our population and address health inequalities.

We are proactive and innovative PCN with a committed team of people who strive to deliver the best care for the patients of our practices and support our practice teams.

By collaboratively working together as 2 Networks, we are able to utilise our resources and workforce to help tackle health inequalities in the surrounding areas with the use of a wider variety of specialist roles.

The 9 practices that form our 2 Primary Care Networks are:

Moreton Medical Centre

Hoylake Road Medical Centre

Hoylake and Meols Medical Centre

Central Park Medical Centre

Liscard Group Practice

The Village Medical Centre

Manor Health Centre

Leasowe Medical Practice.

In addition to the nine practices above we work with wider health organisations such as, Citezens Advice Bureau, Health Junction, Wirral University Teaching Hospital, Wirral Community Trust and Pure Physio.


Job responsibilities

Digital Transformation Support

Using systems such as EMIS, Microsoft Office and local reporting tools to help identify patients who may benefit from PCN services, monitor service activity, review uptake and support more proactive, preventative care.

Assist in the implementation and adoption of new digital systems and processes.

Gather, analyse and present service performance data to support decision-making.

Support delivery of QOF, incentive schemes, QIPP, Investment and Impact Fund (IIF) and other quality or cost effectiveness initiatives.

Help design efficient templates, protocols, alerts and other IT tools to support safety, patient care, quality record keeping and reporting.

Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care

Produce reports, dashboards and presentations for Board meetings and senior leadership teams.

Enhanced Access Service Coordination

Coordinate the operational delivery of the PCN Enhanced Access Service.

Schedule General Practitioners (GPs) and Advanced Nurse Practitioners (ANPs) for evening and weekend clinics.

Ensure clinical staff are appropriately configured within clinical systems.

Set up and maintain appointment books to support service delivery.

Monitor clinic capacity and proactively address scheduling issues.

Complete quarterly service utilisation reports to monitor performance.

Clinical System Administration

Create and maintain appointment books within EMIS and other relevant systems.

Ensure clinician rotas, leave arrangements and service requirements are reflected accurately in appointment schedules.

Set up services in advance to ensure uninterrupted patient access.

Maintain accurate records and system configurations.

General PCN Support

Provide administrative support to managers and clinical leads.

Assist with project work and service development initiatives.

Organise meetings, prepare documentation and record actions where required.

To monitor the PCN mailbox, actioning appropriate items that come in.

To monitor and manage incoming telephone calls, responding to patient queries in a professional and timely manner, and booking patients into appropriate appointments.

To assist with the coordination of various vaccination campaigns (eg Flu & COVID 19) to include stock control, ordering, reporting of activity across the PCN, planning the consent and vaccination of each of our care homes and housebound patients. Scheduling staff for any onsite clinics and managing the booking system.


Person Specification


Experience

* Experience managing competing priorities and meeting deadlines.
* Experience using Microsoft Office applications, particularly Excel, Word and PowerPoint.
* Experience producing reports and handling data.


Qualifications

* GCSE Grade 5 or above (or equivalent) in English and Mathematics.
* Experience managing competing priorities and meeting deadlines.

* Experience managing social media accounts and digital communications.
* Experience working within Primary Care Networks or General Practice.

* Ability to analyse data and present information clearly.
* High attention to detail.
