Key Responsibilities
* Manage post hospital pathways including HomeFirst and step-down beds.
* Manage and attend system calls and provide regular system updates.
* Supervise Social Workers and unqualified staff, providing advice and support to the team.
* Undertake Care Act assessments and Mental Capacity Assessments (MCA).
* Apply safeguarding knowledge to ensure safe and appropriate decision making.
* Work collaboratively with health and social care partners to achieve safe and timely discharges.
* Maintain accurate and up-to-date case records in line with legislation and local authority policy.
Essential Requirements
* Degree/Masters in Social Work and SWE registration.
* 2+ years of post-qualification experience.
* Strong knowledge of UK social care legislation and practice including Care Act, MCA, and Safeguarding.
* Experience in hospital discharge pathways.
* Right to work in UK.
