At HFH Healthcare, we deliver comprehensive, nurse-led complex care to paediatric and adult clients in the comfort and familiarity of their own homes.
As an Onboarding Nurse, you will take ownership of the assessment, planning, and mobilisation of new care packages. You’ll work collaboratively with the client, CHC teams, case managers, internal staff, and families to ensure every new package is set up safely, efficiently, and with quality at the core.

What You’ll Do
5. Complete clinical assessments and mobilisation planning for new packages
6. Oversee the safe onboarding of clients, ensuring carers are trained, competent and well-supported
7. Be visible in client homes during the first 4 weeks of care
8. Support and guide Nurse Case Managers through the onboarding process
9. Build trusted relationships with CHC teams, case managers, and external stakeholders
10. Represent HFH at meetings to promote our model supporting the development of referral pipelines
What You’ll Bring
11. NMC registration (Adult or Children’s Nurse)
12. Experience in complex care, ideally community-based
13. Strong communication, clinical oversight, and planning skills
14. Flexibility to travel and work outside 9–5 as needed
15. Passion for delivering personalised, safe, and responsive care
Requirements
16. NMC registered Nurse
17. Enhanced DBS
18. Full UK driving licence and access to vehicle
19. Flexibility to travel across the Southern Region and work extended hours when needed
