HCT are looking for a registered therapist to bepart of an integrated health and social care team. To provide clinicalleadership and support to the multidisciplinary team in providing care to avoidinappropriate admissions to hospital and support hospital discharge. This willinclude providing assessments, and treatment, where required for vulnerablepatients with potentially complex needs.

To support the Discharge Home to Assess Managerin delivering of high standard therapy service, including service evaluation.
To undertake therapeutic assessment of patients,some of whom may have complex and diverse conditions, and delegate tasks asappropriate.
Following assessment to provide, develop andimplement individual care/treatment plans.
To be part of an integrated health and socialcare team. To provide clinical leadership and support to the multidisciplinaryteam in providing care to avoid inappropriate admissions to hospital andsupport hospital discharge.
To provide assessments, and treatment, where requiredfor vulnerable patients with potentially complex needs. To support patient-ledgoal working within the community
