This is an exciting opportunity to work in the community within a multi-disciplinary, integrated team in the South Kent Coast and Thanet area.
Role Overview
As a Senior Community Learning Disability Nurse, you will be part of a proactive and supportive Learning Disability Nursing Team, enabling and facilitating people with a learning disability to meet their health needs. You will facilitate access to mainstream health services, working collaboratively with primary care, hospitals, and other community providers. Your role involves promoting independence, understanding health needs, reducing health inequalities, and implementing reasonable adjustments.
Responsibilities
1. Manage a caseload with clinical supervision.
2. Line manage Band 5 Community LD Nurses/Nursing Support Workers and support team development.
3. Supervise and assess Nursing Students/Apprentices on placement.
4. Conduct evidence-based assessments and interventions across a range of needs, including end of life care, dementia, sexual health, and health promotion.
5. Develop and deliver group work and educational programs.
Qualifications and Skills
* RNLD/RNMH, RMN, or RGN qualification.
* Minimum of 2 years’ recent experience working with people with Learning Disabilities.
* Good organizational, communication, and interpersonal skills.
* Proactive, dynamic, and committed to flexible service delivery.
* Full, clean driving license and own vehicle are essential.
* Up-to-date knowledge of health needs related to learning disabilities.
* Experience of working with people with learning disabilities for RMNs and RGNs.
