to support the villages commitment in delivering ExtraCares mission of
better lives for older people .
Role Details
Role: Dementia and Mental Wellbeing Enabler
Hours: 20hours per week
Contract: Permanent
Rate: £15,402.40per annum (pro rata FTE £28,957)
Location: Yates Court, High Street, Evesham, WR11 4DN
Extracaredoes notparticipate in the current UK Visa Sponsorship scheme, and we would not be able to facilitate sponsorship.
What you'll be doing
Provideone to one support to older adults living with dementia and common mental health conditions
Working with a multidisciplinary team and being first point of contact when required
Responsible for completing cognitive assessments, mood assessments, Mental Capacity Assessments and Best Interest Meetings
Make sound well-informed decisions while balancing the needs of the residents
Complete risk assessments, and where required raise safeguarding concerns.
What we're looking for
Proven experience in providing support to older adults, particularly those living with Dementia and Mental health conditions
Exceptional communication and interpersonal skills, with the ability to establish rapport with residents, their families and external professionals.
Ability to manage an independent caseload for one-to-one work, deliverance of group interventions, and build connections with external professionals. Respond to changing needs, adapt to new situations, and make quick decisions regarding a residents wellbeing
