What the Job Involves You will join the Community Resource Team (CRT) Homecare in Cardiff, supporting citizens to prevent unnecessary hospital admissions and facilitate discharges. This role involves working closely with health professionals to help service users achieve their reablement goals and live independently in their own homes. You will assess service users to build and implement a plan to support their wellbeing goals. The role includes shift work according to various rotas, including evening and weekend hours, with duties allocated via a mobile device.
