Salary: £23,970 rising to 24,480 after probation Location: Based in our Hwb in: Pontypool - Covering referrals from: Torfaen Contract Typ e: Permanent (in line with funding) - Agile – working where and when required, to do the job most effectively Hours & working Pattern: 35 hours per week (Full Time) • Generally, Monday to Friday with occasional evening and weekend work All team members must work flexibly on occasions to meet the needs of the Charity and the people we support Participate at Tier 2 for the organisational emergency on call system (paid) Accountable to: Community Support Service Coordinator Purpose of the post: To provide specialised, trauma-informed support to Young People ages 16-25 —sometimes with dependent children, who have experienced or been affected by abuse, harm, or violence, within our community services in Torfaen Specific responsibilities • Work collaboratively with team members to provide person-led support to a caseload of adults and/or families, creating safe, respectful environments where individuals feel heard, have choice, and are supported to make informed decisions about their future. • Regularly assess and sensitively manage risk, working in partnership with individuals to promote safety in a way that is inclusive, strengths-based, and enabling. • Support individuals to co-develop personalised support and/or safety plans, ensuring goals are meaningful, achievable, and led by the person’s own priorities and lived experience. • Adhere to safeguarding legislation, policies, and procedures, responding to concerns promptly and appropriately, while maintaining a trauma-informed and non-judgemental approach. • Maintain accurate, timely, and confidential records that reflect the support provided, ensuring documentation is respectful, person-centred, and aligned with best practice. • Provide clear, compassionate signposting to internal and external services, based on the individual’s expressed needs, interests, and readiness to engage. • Create regular opportunities for feedback, ensuring services are continuously shaped by the voices and experiences of those we support. • Contribute to audits and reporting requirements, engaging positively with improvement plans and recognising the value of reflective learning in service development. • Collaborate with team members to ensure the safe and effective running of the Community Support service, including compliance with health and safety standards in a psychologically safe environment. • Actively participate in clinical and managerial supervision, using these spaces for reflection, learning, and maintaining emotional wellbeing. • Support the delivery of adult group and educational programmes, when directed, in a way that is inclusive, empowering, and sensitive to trauma-related needs. • Build and maintain respectful, collaborative relationships with external agencies, championing the needs and rights of those we support.
