Bury Council has a new permanent position available for a Social Worker within the Integrated Discharge Hospital Team to work full-time based in Bury, Greater Manchester.
Location: Bury, Greater Manchester
Working Hours: Full-time, 37 hours per week
Contract: Permanent
Salary: £37,938 – £39,513 per annum

Main Duties as a Social Worker:
* Conduct detailed assessments of patients’ social care needs in preparation for discharge, considering their physical, mental, and emotional well-being.
* Develop personalised, outcome-focused care and support plans that promote independence, recovery, and long-term well-being.
* Involve patients, their families, and carers in decision-making, ensuring their preferences are central to the discharge planning process.
* Work in partnership with hospital staff, community health professionals, and care providers to ensure patients are discharged safely and without unnecessary delay.
* Coordinate and arrange necessary support services, such as home care, residential placements, or community health services, based on the individual’s needs.
* Assess risks to patient safety and well-being in the discharge process and implement appropriate interventions to mitigate those risks.
* Take immediate action in safeguarding cases to protect vulnerable adults, following the appropriate policies and procedures.
Key Requirements for this role:
* Must possess a recognised social work qualification
* Registered with Social Work England
* Experience in hospital discharge or a related setting
* Strong understanding of health and social care legislation, guidance, and policy
* Effective verbal and written communication skills, able to convey complex information clearly to diverse audiences
* Proficient in liaising with families, carers, and multidisciplinary teams
* Competence in maintaining accurate records and using IT systems
