Social care assessor
We have an exciting opportunity for a motivated and enthusiastic individual to join our Adult Social care team as a social care assessor. This is a permanent full time position based at Delamere house within a busy frontline adult social care team.
The post will provide a wide range of learning and training opportunities and includes joint working across the Health and Social Care sector. It will provide you with an excellent opportunity to progress your career and will offer you a wide range of experiences within Adult Social Care services

About the Role:
You will
1. Carry out basic needs-based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response in order to construct an individual Care Plan.
2. Negotiate and liaise with other professionals, statutory and voluntary agencies, relatives and carers about care arrangements in order to maintain the best possible service provision for clients and carers.

About you:
We are looking for someone with-
3. Experience of working with vulnerable people and their carers in a Social Care or Health setting
4. NVQ3, or equivalent experience in Social Care
5. Knowledge of assessment and care management process, including how to undertake needs based assessments
