The postholder will support the multidisciplinary team via social prescribing, helping patients within their homes to build resilience through health and social support.

Work as part of a community-based health team to identify issues affecting health and wellbeing, co-produce care plans, and connect people to community activities and services that meet their needs, including arts, physical activity, and green activities.
Use coaching and motivational interviewing to empower individuals to manage their health, liaise with relevant agencies, and develop strong relationships with providers.
