The successful candidate will primarily be based within one complex care package, while providing clinical oversight and support to a second community based package. Travel to the secondary location will only be required when necessary or to provide cover. Due to the nature of one of the clients needs, this role will require female only staff. About the Clients You’ll be supporting two community-based clients with complex clinical needs within their own homes. Primary Location You will primarily support a client with complex neurological, respiratory and physical health needs, including: Severe neurological impairment Epilepsy and seizure management Complex respiratory vulnerability Regular suctioning and airway management Nebuliser administration PEG feeding and gastrostomy management Nil by mouth enteral feeding Medication administration via PEG Respiratory monitoring Limited mobility and full assistance with personal care This package requires a high level of clinical awareness and confidence in delivering hands-on complex care within a home environment. Secondary Location You will also provide clinical oversight and support, when required, for a second client with: Surgical tracheostomy care Regular suctioning to maintain airway patency Nebuliser administration PEG feeding and enteral nutrition management Medication administration via gastrostomy Monitoring of respiratory status Support with personal care This is a highly clinical, RN-led role requiring strong respiratory awareness, tracheostomy management skills, seizure management knowledge and confidence in delivering complex care within a community setting. The Successful Post Holder Will: Demonstrate excellent verbal and written communication, strong analytical skills and the ability to multi-task. Maintain a high level of attention to detail and accuracy, with pride in clinical documentation and report writing. Provide clinical oversight and governance across both care packages. Support and competency assess care staff where required. Have a genuine passion for delivering personalised, dignified care. Have robust working knowledge of CQC standards and clinical governance requirements. Maintain strict confidentiality and professional boundaries at all times. Be confident working independently within community settings. Work collaboratively with families and MDT professionals. Recognise signs of clinical deterioration and escalate appropriately. Preferred Skills/Qualifications: Registered Nurse with active NMC registration. Minimum 2 years nursing experience within complex care or similar high-dependency settings. Tracheostomy care experience Suctioning experience PEG feeding experience Experience supporting clients with epilepsy and seizure management.
