The post holder will be an autonomous practitioner, working within codes of practice, managing a varied and complex caseload, and providing
highly skilled Occupational Therapy interventions for Monmouthshire residents in their own homes and in-patients at Nevill Hall Hospital, Abergavenny. The goal is to facilitate the maintenance and improvement of their wellbeing to complete activities of daily living, through Reablement and other OT interventions as required. Peers or external references may provide guidance.
The post holder will work alongside other Band 6 Occupational Therapists, providing ongoing input, and will collaborate with multidisciplinary teams in integrated services. Responsibilities include OT assessment and intervention for patients across Monmouthshire to ensure timely discharge, as well as leading tasks such as commissioning and reviewing long-term care and support. The role also involves developing expertise within their clinical specialism.
Main responsibilities include:
1. Practicing occupational therapy safely.
2. Planning and prioritizing workload.
3. Assessing and addressing performance skills in mobility, sensory deficits, intrapersonal and interpersonal skills, self-care, productivity, and leisure.
4. Interpreting patient conditions and providing interventions in complex cases autonomously.
5. Communicating sensitive information empathetically to patients and families, supporting decision-making and goal-setting.
6. Identifying and ordering equipment to maintain independence at home.
7. Maintaining records and documentation per Monmouthshire Integrated Service and COT standards.
8. Attending ward rounds, case conferences, and MDT meetings.
9. Adhering to the COT code of conduct and ethics, including confidentiality.
10. Assessing and managing risks.
11. Developing and maintaining collaborative relationships across disciplines and agencies, including employment and retraining services.
