Job responsibilities
1.Tounderstand and follow good practice within the framework for the Mental HealthAct, Mental Capacity Act and DoLs and Care Programme Approach.
2.To maintainaccurate records, both written and computerised to a standard that reflectstheir level of clinical expertise. Thismay involve the use of specialised psychometric tools to complement theassessment and evaluation of service user care.
3.Liaise withcommunity based services and resources such as Focus, housing, Police andvoluntary sector organisations to facilitate the delivery of the mostappropriate and best possible care for the individual service user, carers andtheir families.
4.Discussissues surrounding risk management and plan care accordingly advising othersregarding management of care with senior practitioners.
6.To respondto service user crisis in a positive pro-active manner.
7.To followclear care pathways, including methods to reduce the risk of further crisisdeveloping.
8.To workflexibly within a 24 hour rota covering early, late and night shifts accordingto service user need.
9.To providetelephone support to any service users, carers, outside agencies or clinicianswho require information, advice or crisis intervention. This will require the Post Holder to makeimmediate, effective decisions that will have implications for the positiveoutcomes of the service users mental health.
10.Undertakecomprehensive CPA mental health assessments then develop and implement carepackages in a variety of settings.
11.To assistin maintaining safe custody of drugs in accordance with the Trust policies andprocedures.
12.Tocover 7 days a week, 24 hours a day on a rota.
Person Specification
Qualifications
* RMN or awaiting Registration.
* Evidence of current registration.
* Basic IT Skills
* Evidence of ongoing professional development since qualification
Knowledge
* Able to respond to service users, carers and significant others in a person centred way.
* Able to maintain a safe environment for service users, carers, staff and others.
* Able to work as a team member and effectively communicate with service users and other professionals.
* Able to assess individuals. Formulate, implement and evaluate care plans.
* Able to participate in clinical supervision.
* Demonstrate and understanding of the Mental Health Act, Care Programme Approach, Mental Capacity Act and DoLs
Experience
* Experience of working with people with mental health problems
* Experience of working with older adults with mental health problems
