Leaving Care Service (Next Chapter)
Are you passionate about supporting our care leavers to get the best possible outcome on their journey to independence? We are focused on our responsibilities as a Corporate Parent and further developing and improving our services to our care leavers. For this role, each personal advisor focuses on the quality of work and relationship‑based practice.
Responsibilities
* Support and shape the improvement journey for care leavers.
* Develop and nurture relationships with young people, recognising their strengths and supporting them to achieve their aspirations.
* Contribute to developing services and influencing policies and decisions that affect care leavers.
* Provide regular supervision and support to grow skills and knowledge within the team.
Qualifications
* Suitable experience as a practitioner working with young people or care leavers.
* Ability to form positive relationships and seek solutions that enable young people to achieve best outcomes.
* Commitment to community values and capability to influence service delivery.
