Investigations are undertaken in line with the Trust patient safety incident response plan and use a systems-based approach.

If you have a background in incident management or investigation and a passion for improving patient care, making a real difference to the people who both use and deliver our services, then reach out for a discussion about the role.

Main duties of the job

The patient safety incident investigator will support the head of patient safety and risk to provide Trust-wide oversight, assuring the quality and effectiveness of all types of patient safety incident investigations as part of the Trust’s annually agreed Patient Safety Incident Response Plan (PSIRP). The patient safety incident investigator will undertake robust patient safety incident investigations by working with a range of internal and external stakeholders to investigate, analyse and use judgement through the collation of evidence and findings using the agreed methods within the Trust’s PSIRP.
