Job summary
An exciting opportunity to be part of an innovative award-winning organisation, integrating the clinical skills of a pharmacist into the general practice/PCN team. We are looking for enthusiastic, motivated pharmacists committed to developing their knowledge and skills to improve patient care in general practice.
Main duties and responsibilities
* Act as an experienced pharmacist within a multi-disciplinary General Practice team, providing clinical pharmacy support to achieve PCN/Practice priorities.
* Provide support to PCN/practice staff on prescription and medication queries, ensuring integration with community and hospital pharmacy to optimise skill mix and improve patient outcomes.
* Contribute to service development, information management, medicines quality improvement and medicines safety; support education and training activities.
* Engage in patient-facing clinical activities such as medication reviews, medicines support, and telephone medicines advice as appropriate to the role.
* Signpost and support patients to appropriate services to improve access and care delivery.
Qualifications and experience
* Minimum of 5 years post-registration experience.
* Evidence of further education in clinical pharmacy essential (e.g., NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma).
* Evidence of CPD relevant to General Practice is essential.
