Community Navigator, Intermediate Care (Hospital@Home) Service
Duties and Responsibilities:
* Provide comprehensive administrative and secretarial support to the Intermediate Care (Hospital@Home) Service, including managing diaries, emails, and composing routine correspondence daily.
* Utilize available technology such as Microsoft Office, Civica scheduling software, and ESR to support administrative tasks.
* Plan and prioritize workload, adjusting meeting arrangements as needed, and handle conflicting diary appointments using judgment for alternative planning.
* Assist in maintaining and updating team training programs and rotas, ensuring distribution to relevant parties.
* Arrange staff training sessions, including scheduling, notifications, and database entry.
* Organize team meetings and facilitate team activities.
* Manage confidential patient information, ensuring appropriate communication with departments, agencies, patients, and carers.
* Seek advice/support for responsibilities outside own scope, and appropriately archive notes and documentation following policy.
* Support HR functions such as organizing PDRs, sickness reviews, and managing staff leave records.
* Assist with drafting and updating staffing rotas and communicating changes.
* Coordinate with help desks to resolve operational issues.
* Maintain office systems, including filing and communication boards.
* Handle calls professionally, responding to inquiries or referring them as necessary.
* Maintain confidentiality and handle sensitive information appropriately.
* Utilize office equipment responsibly and order supplies as needed.
* Attend staff meetings and support training for new team members.
* Validate invoices and produce reports and statistical analyses of team activity.
* Assist with research and audits, and complete staff surveys as required.
* Identify training needs and attend study days.
* Report potential or actual problems to the Line Manager.
* Work independently within policies and procedures, providing cross cover for colleagues during absences.
Note: Work is based at the Intermediate Care (Hospital@Home) Service location, with potential to work across the locality based on service needs.
