Livewell Southwest is an independent, award-winning social enterprise delivering integrated health and social care services. We are seeking a Community Registered Nurse to join our team.
The role requires positivity, excellent communication, leadership, and caseload management abilities. You will need to demonstrate integrity, initiative, sound clinical knowledge, and the ability to support the team.
We work integrated with primary care, adult social care, therapists, urgent, and intermediate care services. We strive to improve health outcomes and patient experience.
Main duties of the job
As a Community Registered Nurse, your role is vital in terms of coordinating people's care through holistic evidence-based best practice. You will be an autonomous practitioner and role model, responsible for assessing, triaging, planning, implementation, and evaluation of care.
You will support the Community Sister/Charge Nurse in their absence and manage sensitive issues. You will be required to undertake a robust induction, and a mentor will be allocated to support your learning and development.
Person Specification
Essential:
* Experience of a variety of healthcare settings
* Knowledge of recent developments within health and social care
* 1st Level Nurse, Registration (degree or diploma), and NMC Registration
* Ability to access/travel to patient homes across the locality
* Ability to use initiative and work autonomously
* Ability to decision make and problem solve
Desirable:
* Previous community nursing experience
* Advanced knowledge and competence with specialized skills required in everyday community nursing
* Experience of managing a caseload
