Were looking for a compassionate and proactiveHealthcare Assistant to join our Integrated Neighbourhood Team at Banbury CrossHealth Centre. Youll play a vital role in supporting complex, vulnerable, andoften housebound patients across the Banbury area. This is a dynamic andrewarding position where youll manage a dedicated caseload, conduct homevisits, and contribute to holistic, patient-centred care.

About our team

Our Integrated Neighbourhood Team is acollaborative, multi-disciplinary team (MDT) including GPs, Care Coordinators,Social Prescribers, and other healthcare professionals within the wider primarycare team at Banbury Cross Health centre.

Our mission is to reduce avoidable hospitaladmissions by proactively managing patients in the community. We identifypatients with increasing frailty and address health inequalities within ourdeprived patient population. We take referrals from health care professionalsand proactively review patients post hospital discharge who meet our criteria.

We work in partnership with District Nurses,community therapy services, PML neighbourhood frailty and visiting teams,social workers, Hospital at Home, and Geriatricians to deliver integrated care.We also work with community and voluntary organisations such as Age UK,Dementia Oxfordshire, Cherwell District Council, and the Volunteer TransportService to ensure our patients receive comprehensive support.

This role may close early if sufficient applications are recieved.


Main duties of the job

As a key member of our MDT, youll:

* Deliver care to patients in their homes acrossBanbury (business-insured vehicle and full UK driving licence required)
* Support clinical colleagues with patient reviewsand care planning
* Perform clinical tasks such as phlebotomy, ECGs,blood pressure checks, and simple dressings
* Recognise signs of deterioration and escalateconcerns appropriately
* Support patients and families with increasingfrailty
* Monitor long-term conditions and promote healthprevention and screening
* Attend and contribute to MDT meetings
* Maintain accurate patient records and assistwith referrals
* Uphold infection control standards andcontribute to clinical governance

PML is a successful not-for-profit, GP-led organisation providing various NHS community and primary care clinical services to patients across Oxfordshire and Northamptonshire. We have evolved as a NHS healthcare provider since 2004 and in the last few years have grown significantly, now employing around 300 staff with a turnover of circa £16m. PML holds GMS contracts, as well as being a GP Federation representing circa 50 GP practices covering approximately 650,000 patients.

Administrative & Technical Skills

* Proficient in MS Excel, Outlook, Teams, andPrimary care IT systems such as EMIS, Blinx, Accurex, Docman etc (training willbe provided)
* Able to document MDT minutes and supportadministrative coordination
* Familiar with data protection and clinicalgovernance standards

* Ability to use basic physiological recording equipment and accurately record findings.
* Ability to demonstrate care and compassion and commitment to/for patients and carers.
* Ability to demonstrate a clear desire to work in general practice and a clear understanding of the role of a HCA
* An understanding, acceptance and adherence to the need for strict confidentiality.
* Able to work on own initiative.
* Able to work under pressure and remain calm.
