Job Summary
To care for patients with palliative care needs as requested by St David's Nurses. To provide support and respite care for patients and their families in their own home. Night shift.
Main duties of the job
1. Work in accordance with the policies and procedures of St. Davids Hospice Care and CIW (Care Inspectorate Wales).
2. Maintain confidentiality at all times.
3. Ensure patients' comfort and dignity at all times.
4. Work alongside patients and carers, respecting their privacy.
5. Report changes in patient condition to the appropriate Clinical Nurse Specialist.
6. Assist with personal hygiene and toilet needs if appropriate.
7. Maintain a safe environment for self, patients, and carers.
8. Monitor symptoms and take appropriate action during shifts.
9. Administer prescribed medications.
10. Be familiar with the Care Decisions Tool for the last days of life.

Job Responsibilities
General Responsibilities
1. Recognize and accept the limits of personal knowledge and responsibility.
2. Be aware of responsibilities towards Health and Safety Regulations.
3. Report accidents or incidents verbally and in writing.
4. Follow procedures to contact the out-of-hours service when advice is sought.
5. Observe a No Smoking policy while on duty.
6. Observe a No Sleeping on duty policy.
7. Undertake necessary training.
8. Attend clinical supervision and hospice at home meetings.
9. Log in and out of Guardian to confirm arrival and departure at patient’s house.
10. Contact the Nurse in charge if problems occur during shift or if patient’s condition causes concern.
11. Ensure a verbal handover from the clinical team before shift begins.
12. Be aware of and act according to risk assessments for each patient.

Qualifications
* RGN
* NMC Registration
* Competent in managing syringe drivers
Desirable
* Community experience
* Certificate in Palliative Care / Familiarity with the Care Decisions Tool for the last days of life
Experience
* Experience caring for patients and families needing palliative care.
* Community experience.

This role is subject to a DBS check and current UK professional registration.
