An exciting opportunity has arisen for an enthusiastic and motivated individual to join the South East Essex Virtual Frailty Ward service. This important post will work alongside a team of Consultant Geriatricians, Community Nurses, Therapists, and Health Care Assistants in the provision of the Frailty Virtual Ward working in the community, in the patient’s own home.
The service will reduce demand for inpatient beds and on Emergency Departments and optimise patient experience by enabling them to receive safe clinical specialist care in the comfort of their own home.
