Children Community Learning Disability Nurse
We are seeking 2 highly motivated Registered Learning Disability Nurses to join the county wide Children and Young People’s Learning Disability Service in Surrey. The service operates across four bases: Redhill, Leatherhead, Godalming & Chertsey. CLD is a nurse-led service with Psychology / APs / Psychiatry / Music Therapy / Nurses and admin support. The team works collaboratively with families, carers and the young person to establish a plan of care, focusing on behaviour and anxiety related needs. The service uses a Positive Behaviour Support (PBS) framework and runs termly Behaviour Clinics with nurses and APs. CLD collaborates with education, school nursing, social care, paediatrics and other agencies supporting the young person. There are regular meetings, supervision and appraisal, with opportunities for career development. You will need to be a driver with access to a car as the role covers a geographical area across Surrey and involves visiting family homes, schools and other locations. Attendance at training and meetings at Leatherhead HQ is required.

Responsibilities
* Hold a caseload of children and young people in the community, following a Discovery Conversation to identify presenting needs.
* Demonstrate specialist nursing skills in learning disability areas such as crisis intervention, communication support, assessment and management of additional mental health needs, epilepsy, medication management and challenging behaviour.
* Act as an advocate to involve people in developing and agreeing their own care plan, ensuring dignity, choice, self-esteem and respect for cultural and religious needs.
* Carry out comprehensive assessments, plan, implement and evaluate nursing interventions as part of a holistic approach in varied settings.
* Practice within the Positive Behaviour Support framework.
Qualifications
* Registered Learning Disability Nurse with knowledge of PBS.
* Experience of working with children is ideal; transferable skills from adults to children are welcome.
* Car driver with access to a car required for travel across Surrey; willingness to attend training and meetings at Leatherhead HQ.
* Knowledge of I Thrive Model is desirable.
