Main Duties of the Job
* A fully qualified nurse with at least 12 months experience
* Deliver safe, holistic, evidence-based nursing care for patients with complex palliative needs, working closely with our wider MDT (medical, pharmacy, AHPs, family support, and volunteers).
* Assess, plan, implement and evaluate individualised care plans focused on comfort, dignity and patient/family goals.
* Administer and monitor medicines in line with hospice policies and NMC standards, escalating changes in condition appropriately.
* Communicate sensitively with patients and those important to them, promoting shared decision‑making and advance care planning.
* Maintain accurate, timely documentation on our electronic patient record system and contribute to quality improvement, incident reporting and learning.
* Support safe discharge planning and smooth transitions of care across services.
Experience
* Provision of care in an acute setting
* Evidence of professional development
* Have provided palliative care to patients in other settings
Key Skills & Abilities
* Demonstrate proficient skills in written and verbal communication in the English language
* Basic understanding of drugs commonly used in palliative care/end‑of‑life care
* Evidence of professional development, self‑motivation and initiative
* Professional, sensitive style of communication and possess good IT skills
* Able to prioritise and manage workload effectively whilst working flexibly and as part of a team
* Have the emotional resilience to work effectively in a palliative care setting and cope with care of patient who are dying and their care after death
* Good moving and handling skills
* Maintain confidentiality at all times and ensure colleagues uphold the same standard
* Promote equality and diversity
* Good knowledge of pain and symptom management
* Good understanding of health and safety issues in a clinical environment
* Have a good understanding of the Mental Capacity Act 2005
Qualifications
* Registered Nurse on NMC register with at least 12 months experience
* Additional qualifications in health/social care at diploma level
