Health Coach - Healthy Lifestyle Service
Enfield Unity PCN are looking for an enthusiastic & dedicated health coach to work with our Personalised Care Team, meeting the needs of local communities to reduce health and social inequalities.
Main duties of the job
* Motivate and empower patients to make behaviour changes around diet, physical activity and smoking cessation.
* Support service users to develop healthier eating habits and balanced meals.
* Promote and support weight loss or prevent further excess weight gain in service users living with overweight or obesity.
* Offer smoking cessation support and basic health checks (height/weight/BP).
* Deliver one‑to‑one and group stop‑smoking support sessions and other healthy‑lifestyle support, ensuring access for local‑community clients.
* Deliver a minimum of three full‑length consultations (face‑to‑face or remote, as per patient preference).
* Plan and organise workload, including caseload management.
* Provide a high‑quality, efficient service across diverse community settings.
* Monitor outcomes of treatment programmes for effectiveness.
* Manage conflict effectively in both group and individual settings.
* The advisor may need to work with complex health needs related to behaviour change.
* In partnership with the service coordinator and manager, develop new service initiatives based on periodic impact‑needs assessments.
* Develop and maintain close working relationships with key stakeholders and partners.
* Ensure personal development by staying updated on research and best practice.
* Attend team meetings and training as required.
* Actively participate in promotional events.
* Carry out any other duties related to this post.
About us
Enfield Unity PCN is comprised of 17 GP Practice sites across Enfield.
Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation. MHP is a super‑partnership made up of 9 of the 17 PCN sites.
The successful applicants will have access to the training & support offers of this large organisation.
The successful applicants will be working closely with the PCN Personalised Care Team, Clinical Directors and Clinical Leads.
Person Specification
Knowledge and Skills
* Knowledge of behaviour‑change models and the problems experienced by socially disadvantaged sectors of the community.
* Understanding of the impact of obesity and smoking on health.
* Knowledge of dietary approaches represented by the Eatwell Guide.
* Knowledge of up‑to‑date physical‑activity recommendations and strategies to reduce sedentary behaviour.
* Working knowledge of information‑governance issues and safeguarding processes and policies.
* Excellent time‑management and organisational skills.
* Ability to work independently or as part of a team.
* Commitment to provide an efficient and effective service.
* Competent IT skills, including Microsoft Office.
* Knowledge of stop‑smoking and tobacco‑control policies.
Qualifications
* Qualified in health coaching or an equivalent qualification.
* Knowledge of healthy‑lifestyle behaviour change as a specialist field, acquired through relevant experience or further education.
* NVQ level 2 (or equivalent) in Health and Social Care.
* Completed the Care Certificate.
* Qualified in nutrition.
Experience
* Experience working in an environment that promotes behaviour change.
* Experience working with clients from a variety of social and cultural backgrounds.
* Enthusiastic and self‑motivated attitude.
* Ability to adopt a flexible working pattern.
* Experience working in a GP Practice.
* Experience with Emis Web.
