Job Title: Population Health Coordinator

Location: Riedman Campus 100 Kings Highway South
Hours Per Week: 40
Schedule: Monday - Friday 8 am - 4:30 pm
SUMMARY
Key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health.
RESPONSIBILITIES
* Documents all patient interactions in a concise manner that is compliant with documentation requirements
* Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management
* Assists to schedule and coordinate patient transportation, follow-up physician appointments
* Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff
* Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports
* Provides excellent customer service by serving as a resource to all internal and external customers
* Maintains knowledge of all health plan benefits and payor requirements,
* Call patients as designated by PCP or clinical staff
* Verify patients have been seen and follow up on no show patients
* Assist all patients with barriers
* Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of care
* Coordinates other medical office assistants in outreach and report validation needs
* Closes Care Gaps
Minimum Qualifications
* High school graduate or GED equivalent preferred
* More than one year of related experience in a medical setting
* Basic computer literacy
* Knowledge of medical terminology
* Good communication and customer service skills
* Able to perform all Medical Office Assistant job functions and responsibilities
Required Licensure/Certification Skills
* Valid NYS drivers’ license
Preferred Licensure/Certification Skills
* Current Medical Assistant certification or registration preferred

PHYSICAL REQUIREMENTS
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE
$19.85 - $21.50
