Woking & Sam Beare Hospices and Wellbeing Care are pleased to announce the following position:

Band 7 Clinical Nurse Specialist – Community Palliative Care

We are a patient centred charity that delivers palliative and end of life care to thousands of people each year with advanced life-limiting illnesses who live in Northwest Surrey. The Hospice also provides support to their carers and families in patient homes, in care or nursing homes, or through its Inpatient Unit in the Hospice, as well as though the Bradbury Wellbeing Centre. This role is an essential component in supporting and complementing the Hospice Community Services Team, ensuring that the highest standards of care are implemented and that services meet statutory regulations and national standards.

Clinical Colleague

A successful Community Clinical Nurse Specialist will be able to:

1. Provide a high level of clinical expertise, to support and facilitate individualised patient-centred care to patients with life-limiting illness and to provide care and support for relatives and carers, in collaboration with the multi-disciplinary team.
2. Facilitate and support the implementation of evidence-based practice and contribute to the on-going development of the service.
3. Participate in a seven day per week operational service.
4. Demonstrate leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care.
5. Identify needs through a holistic assessment and co-creating a care and support plan with people living with life-limiting illnesses.
6. Independently manage a varied caseload of patients and associated responsibilities.
7. Work autonomously and within a multidisciplinary team.
8. Assess, plan, implement and evaluate as aspects of patients care.
9. Motivate and inspire others
10. Form and maintain professional working relationships
11. Have good working knowledge and application of MS Office packages

A successful candidate will have:

12. A legal right to work in the UK. We are not able to offer sponsorship.
13. Registered Nurse on NMC register
14. Minimum 4 years relevant post-registration experience or significant palliative or oncology experience
15. Experience of ongoing professional development
16. Mentorship qualification – ENB, Mentorship, Practice Assessor/ Supervisor
17. Car driver / full driving licence
18. Access to a vehicle for community visits and ensure appropriate insurance in place
19. Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Training & Development: We have a dedicated education and training team to support development opportunities across the hospice including apprenticeships, professional qualifications and more.
