This locum position offers an hourly rate of £27 on an ongoing basis. You'll be working three days a week, with shifts scheduled between Monday and Friday from 9 am to 5 pm. What you will do:- Take on the crucial role of care coordinator for a small caseload of clients, ensuring each receives tailored and effective care.- Conduct thorough assessments of care needs, develop innovative recovery-based care plans, and oversee their implementation and evaluation.- Collaborate with a multidisciplinary team to provide comprehensive support and treatment for individuals experiencing early psychosis.- Engage with patients and their families, offering guidance, education, and support throughout the recovery process.- Stay updated with the latest practices in early intervention and apply this knowledge to improve patient outcomes.
