Overview
The CRHTT operates a 24 hour a day, 7 day a week service for people aged 18 years and above who are experiencing a mental health crisis or requiring support to be discharged from the acute wards. The service will maintain focus on identifying, assessing and care planning for service users. The post holder will be expected to be self-motivating with the ability to demonstrate accurate formulations of mental state, risk and need for people in a mental health crisis and take into account medical, social and cultural factors, referring on to other disciplines in the team or other agencies as appropriate within the given time frames for the service. The post holder will assess and update risk on each contact and be fully aware of safeguarding concerns. The post holder will provide advice on mental health issues and participate in carrying out training programs for clinical teams on the management of a wide variety of mental health problems. The post holder will maintain the highest degree of professionalism and service at all times and will work in collaboration with the teams and organisations that come into contact with the service.
Responsibilities
* Assess referrals from GPs, CMHTs and acute admission wards and provide home treatment for patients accepted onto the caseload.
* Be part of the assessment or home treatment team within the Crisis Team.
* Provide Gatekeeping and feedback based on the CRHTT criteria.
* Provide comprehensive risk assessments incorporating baseline physical assessments.
* Provide expert advice on management, treatment interventions and diagnosis of patients experiencing a mental health crisis to minimise risks, ensure safe care and promote recovery.
* Support acute hospital wards and facilitate early discharge of patients or support them while on leave to reduce length of stay.
* Use a range of psycho-social interventions to improve mental health outcomes for patients.
* Reduce readmissions to acute hospitals by facilitating client education and service-user led recovery and longer-term support.
* Provide seamless care from GP or ward to community for patients under our service.
* Act as shift coordinator, assigning colleagues' work for the day.
* Be aware of and adhere to lone worker policy.
* supervise and facilitate the administration of medication when required.
* Participate in multidisciplinary team meetings as well as team and business meetings.
* Promote social inclusion, community access and participation through client care.
* Act as an advocate for patients and carers where appropriate and with authorisation.
Qualifications and Experience
* Experience working within a multidisciplinary service.
* Experience of risk assessment and safeguarding concerns in a mental health context.
* Ability to provide psycho-social interventions and work within an integrated care framework.
