Health Care Assistant - Home First

The closing date is 19 May 2026

Do you want to help people in your local community regain their independence following an accident or illness, and to be able to get the most from life?

Do you want to be part of a dynamic NHS team where no two days are the same, but every day brings rewards in terms of job satisfaction and team working?

Do you want flexibility with the opportunity to develop skills and knowledge with opportunities for career development?

If you are enthusiastic and have experience in delivering reablement (short‑term care that helps someone become more independent), therapy or other care and want to make a positive difference to people’s lives in your local community, we want to hear from you!

This is an exciting opportunity to be part of Wirral’s Community Integrated Response Team, where you will support people to remain independent in their own homes, using a reablement approach to help them reach their full potential.

Daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation, and social care and support. You will work with therapists and social workers to ensure people achieve their recovery goals.

Care is provided to patients within their own homes, so a valid driving licence and suitable vehicle are essential.
