This is an exciting opportunity to be part of Wirral’s Community Integrated Response Team, where you will support people to remain independent in their own homes, using a reablement approach to help them reach their full potential.
Daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation, and social care and support. You will work with therapists and social workers to ensure people achieve their recovery goals.
Care is provided to patients within their own homes, so a valid driving licence and suitable vehicle are essential.
Main duties of the job
To join our multidisciplinary team you will:
* Have an NVQ Level 3 in Health and Social Care, or demonstrating equivalent experience
* Display a caring attitude
* Have a strong desire to help people
* Have good communication skills
The post involves a 7‑day working rota.
About the Community Integrated Response Team (CIRT)
CIRT is an integrated team of clinical staff and professionals including nurses, physiotherapists, occupational therapists and social workers.
The team focuses on admission prevention and early supported discharge from ED and assessment areas. The Healthcare Assistant role supports patients with reablement and self‑care, encouraging them to reach their optimum level of independence.

This post requires good communication, interpersonal skills and the ability to be adaptable, reliable and approachable.
We are looking for candidates with experience of providing reablement, therapy and/or care in the community and working as part of a multi‑disciplinary team.
The job requires working over a 7‑day shift pattern, 8 am – 10 pm, Monday – Sunday.
Job responsibilities
* Implement assessed care needs and help maintain progress in rehabilitation / reablement.
* Assist patients with personal care tasks delegated as part of the reablement care plan, plus additional duties such as meal and drink preparation.
* Provide care in people’s homes, working with individuals and families to ensure a person‑centred approach, including participation in clinical care procedures such as recording observations and applying topical cream.
* Support admission prevention by responding to incidents of carer breakdown and providing rapid support to prevent admission to hospital.
* Assist with medication administration as per care plans and policies.
