Salary: Up to £28,000 per annum (depending on skills and experience)
Role Overview:
Our client is seeking a dedicated Resettlement Support Practitioner to provide peripatetic cover across the East of England (Kent), supporting participants transitioning from custody to the community. You will manage a caseload of individuals, delivering tailored guidance and high-quality support to address barriers to resettlement. The goal is to help participants reach key milestones, develop their skills, and move towards sustainable employment.
Key Responsibilities:
Caseload Management: Manage a caseload of participants with complex needs, ensuring the coordination and delivery of services such as induction, assessment, support planning, training, education, pre-release preparation, and employment support.
Through-the-Gate Support: Provide consistent community-based support to participants leaving custodial settings, ensuring a seamless transition and continuity of care.
Case Management & Compliance: Maintain accurate and up-to-date case records using the CATS+ case management system, ensuring achievements and progress are recorded in line with compliance standards to avoid rejected submissions.
Prison Records Update: Update P-Nomis, the prison case management system, with relevant participant information, including actions taken, achievements, and any safety or security-related updates.
Stakeholder Relationship Management: Build and maintain effective relationships with local stakeholders, both in custodial settings and the wider community, to facilitate referrals and avoid service duplication.
Collaboration with CFO Evolution Team: Work collaboratively with internal and external CFO Evolution colleagues to ensure smooth transitions for participants. This includes arranging pre-release 3-way meetings and supporting with Release on Temporary Licence (RoTL) where appropriate.
Partner Coordination: Assist in identifying, resourcing, and coordinating partner delivery services to meet the needs of CFO Evolution participants.
Community Engagement: Support the delivery of services within CFO Evolution Community Pop-Up Hubs, enhancing access to local support and opportunities

Essential/Desirable Criteria:
ICT proficiency including all major Microsoft applications
Level 2 English & Maths or equivalent experience
Understanding of the barriers faced by prisoners/prison leavers with multiple and/or complex needs in relation to reintegration back into society
Ability to work with and manage challenging behaviour
