Family Wellbeing Practitioner (Young Person) Project Worker 1: for Barnardo's Cyfle Service.

Location of Post:

The Young PersonWellbeing Practitioner will be based from the Westbourne Centre in Rhyl and will supporting young people in local schools/community hubs and family homes. A digital offer will also be made available to the service users.

Summary of Service:

Service 2- Young Person's Mental Health and Emotional Wellbeing Service.

Aim: To offer 1:1 and/or group support to young people to improve their emotional wellbeing through tailored support packages which is reviewed on a regular basis to ensure their voice is heard in the progress of their plan- to help them reach their desired outcomes.

Who is it for: Young People aged 12- up until their 18th Birthday (depending on assessed need).

Job purpose

The post holder will directly support young people to positively manage emotional well-being issues.

Job Activities:

To meet with the young person and support with their emotional health and wellbeing needs on a regular and planned basis. An assessment of their needs will be required to help formulate a support plan.

1. To facilitate communication and engagement with young people using a range of approaches.
2. Attend service meetings involving staff and young people.
3. Liaise, mediate and advocate with other agencies, individuals and communities who are significant to the lives of young people.
4. Encourage active participation of young people in the management, operations and development of the service.
5. Work with young people, the Voice and Influence worker and the Team Manager and Children's Services Manager to facilitate young people presenting their views and ideas directly to help shape the development of the new service.
6. Contribute to meeting and reporting on targets set by Families First.

Specific Essential Requirements:

7. Knowledge of other supportive emotional health and wellbeing services available to young people.
8. Knowledge of issues affecting young people and the systems which impact upon their lives (Health, benefits, education, employment, housing).
9. Involvement in establishing and working with support networks for young people.
10. Involvement in designing and implementing group and individual programmes of work with vulnerable young people.
11. Skilled in communicating with young people.
12. A full, clean driving licence and the use of a car to meet the requirements of the post.
13. Be able to work flexible hours, including planned occasional evenings and weekends.
