Overview
An opportunity has arisen for 2 x Senior Occupational Therapist (OT) within the Independent Living Service. One post will be based within the main area of the Independent Living Service and the other will be based in the Discharge to Assess function of The Independent Living Service. Both posts are permanent. The service supports hospital discharge, community follow-up, and specialist input for people with disabilities across Bolton, working closely with Community Therapy and Social Work teams.
In this role, you will support safe hospital discharges and follow-up care in the community, ensuring individuals have the right support to avoid readmission. You will carry out Care Act 2014 assessments, develop support plans, and apply relevant legislation such as the Mental Capacity Act 2005. Working closely with therapy and social work teams, you will assess needs, provide equipment, and promote independence. You will also contribute to service development, improve user and carer experiences through person-centred planning, and support integrated care through collaborative working.
Responsibilities
* Support safe hospital discharges and follow-up care in the community.
* Carry out Care Act 2014 assessments and develop suitable support plans.
* Apply relevant legislation such as the Mental Capacity Act 2005.
* Assess needs in collaboration with therapy and social work teams, provide equipment, and promote independence.
* Contribute to service development and improvements in user and carer experiences through person-centred planning.
* Support integrated care through collaborative working with partners.
* Work on a 7% weekend/bank holiday enhancement due to rotational shift requirements to deliver the service 365 days a year (as part of a 5 from 7 arrangement).
Requirements / Qualifications
* Relevant qualification and registration with Social Work England.
* Full UK driving licence; essential car user allowance included.
* Ability to work in a designated customer-facing role under the fluency duty with a specified level of spoken English as per the person specification.
Role Details
Grade: I SCP 31 to SCP 35 (£41,771 to £46,142).
Contract: Permanent — 2 x 37 hours per week (Mon–Fri and 5 days from 7); 7% enhancement for shift working.
