What you will do
* Provide an outcome‑focused assessment service for adults with physical disabilities or age‑related challenges, supporting their journey towards greater independence.
* Collaborate with local authorities and hospital teams for smooth discharge processes and continuity of care.
* Utilise the Mosaic case management system for efficient and effective case management, ensuring best practices are maintained.
* Travel throughout the area as a driver with access to a personal vehicle to visit clients and liaise with different agencies.
Requirements
* Must be a registered HCPC Occupational Therapist.
* Possess a valid driving licence and have access to a vehicle for work‑related travel.
