Overview
Provide specialist nurse‑led TB services across the Trust to reduce the impact of TB. Deliver high‑quality, individualised care to patients, clients and carers without supervision, collaborating with the multidisciplinary team and acting as a role model for clinical practice. The role involves deputising for the line manager, managing complex situations, and covering all three sites (Northwick Park, Ealing and Central Middlesex Hospitals).
Responsibilities
* Act as a resource for patients and the clinical team by providing comprehensive advice, information, teaching and training within LNWUHT.
* Case‑manage patients with latent TB and a small cohort of active TB cases.
* Identify and record all tuberculosis cases in collaboration with the Health Protection Agency and the NTBS notification systems.
* Contribute to cohort review and case conferences, and to incident meetings.
* Act as the nurse on call and triage TB referrals.
* Establish links with community groups, general practices, voluntary organisations and other community workers to ensure equitable and culturally sensitive TB screening.
* Liaise closely with UK HSA, Find and Treat, and with the UK HSA regarding the epidemiology of tuberculosis and the management of specific cases.
* Carry out and interpret TB and HIV related tests (e.g., Tuberculin skin test, IGRA, Pre/Post HIV test discussion, viral hepatitis, blood tests, Ishihara test, visual acuity and BCG) following agreed protocols.
* Provide telephone advice to GPs regarding referrals and patient management, and to patients and the public about TB exposure, symptoms, side effects and results.
* Provide nurse‑led clinics and domiciliary visits to adults and children, including those co‑infected with HIV.
* Investigate TB cases/outbreaks, perform risk assessment, contact identification, tracing and screening, and coordinate mass screening with UK HSA under the direction of a Band 7 nurse.
* Identify clinical risks and ensure appropriate action is taken in accordance with Trust Risk Management policy, working collaboratively with other agencies to prevent nosocomial spread of TB.
* Contribute to the development of the TB service locally.
