Paediatric Long Term Conditions Nurse (Epilepsy)
Our Children\'s Community Nursing Service has an exciting development opportunity in the role of a Paediatric Long Term Conditions Nurse for Epilepsy. The role provides clinical nursing expertise to children and young people (CYP) with epilepsy across the Health Board, aims to enable self-care, and includes education for families, carers and other professionals.
Overview
The successful applicant will possess specialist clinical skills to achieve the best outcomes for this cohort of CYP. The post involves managing an autonomous caseload and supporting the team to deliver person-centred care in hospital, home and school settings.
Responsibilities
* Manage own defined caseload, work autonomously and support the team to achieve person-centred care.
* Assess, plan, implement and evaluate holistic epilepsy nursing care in the hospital, home and school setting.
* Work in partnership with families, empowering parents and children to ensure that epilepsy is managed in the community, reducing hospital stays at diagnosis and minimising readmission.
* Provide specialist teaching for children, young people and their families, facilitating the development of self-care skills and knowledge, at diagnosis and in ongoing age-appropriate education both individually and in groups.
* Work in partnership with the nominated paediatrician on the implementation of individual pathways of care.
* Develop and deliver specialist education programmes for nursing and other professionals across all care sectors including education, as well as patients and their families.
* Provide specialist nursing guidance and ongoing support to all nursing teams involved in patient care.
* Support and participate in multidisciplinary service development, acting as an advocate for the child and family in the coordination of all aspects of patient care.
Requirements / Qualifications
* Specialist clinical skills in paediatric epilepsy care and education for patients and families.
* Ability to work autonomously and collaboratively within a multidisciplinary team.
* Desirable: ability to speak Welsh; English and/or Welsh speakers are welcome to apply. Welsh language support/learning options are available during employment.
