Key Responsibilities Provide long-term condition management, including reviews and care planning for patients with asthma, COPD, CVD, and diabetes. Deliver womens health services, including contraception, cervical screening, and health promotion. Undertake wound management, dressings, and immunisations (childhood and adult). Support patients with lifestyle advice, health education, and preventative care.
Work collaboratively with the wider clinical team, including GPs, HCAs, clinical pharmacists, and allied health professionals. Maintain accurate clinical records in line with professional and practice standards. Participate in quality improvement activities, audits, and practice meetings. Support the training and supervision of students and junior staff, as appropriate.
