About this role
We are looking for a highly motivated individual to join our Dementia Liaison Nurse (DLN) service. The post forms part of an eight‑person innovative DLN team in Cornwall and the Isles of Scilly, delivering evidence‑based dementia care in designated care homes and collaborating with the wider community, general practice, and social care.
Key responsibilities
* Assess, risk assess and manage people living with dementia, including formulation and review of care plans.
* Conduct specialist bio‑psycho‑social assessments, monitor physical health and medication impacts, and carry out carer and cognitive functioning assessments.
* Work collaboratively with the immediate multi‑disciplinary team within the Dementia and Older People’s Mental Health service and with wider health and social care partners, including GP practices, care homes and community teams.
* Help reduce admissions from care homes to acute hospitals (Royal Cornwall Hospital and Derriford) by placing the person at the centre of care within a caseload under the care programme approach.
* Maintain good communication with carers/family members and community services, applying the Mental Capacity Act and Mental Health Act and understanding the physiology of ageing and different types of dementia.
Qualifications
* Experience and understanding of using a bio‑psycho‑social approach to meet the needs of people living with dementia.
* Sound knowledge of the Mental Capacity Act and Mental Health Act.
* Strong communication skills and the ability to work independently as well as within a wider team.
* Current employees of Cornwall Foundation Trust, Royal Cornwall Hospitals NHS Trust, or Cornwall and Isles of Scilly Integrated Care Board (only NHS employees in Cornwall or the Isles of Scilly may apply).
