Job Summary
Blackheath Medical Centre is a small patient focused training practice looking to recruit a friendly, enthusiastic care navigator on a temporary contract for 6 weeks to start immediately.
We offer a range of consultations such as online, telephone and face to face appointments. The practice is forward thinking and offers a high level of care to patients and a high achiever in QOF.
Key responsibilities of Care Navigator include:
· Answering the telephone and dealing with patients at the front desk
· Sign post patients to the most appropriate service
· Request blood and prescription requests
· Deal with queries from patients and other services via telephone and emails
· Booking patients onto EMIS clinical system (in practice and cross organisation services)
· Arrange services to support our more vulnerable patients – referral to social prescriber and arrange interpreter services
The ideal successful candidates will have a combination of:
· Knowledge of EMIS, Docman, AccuRx Patchs and
· Be a team player and be able to communicate both verbally and written form
· Excellent customer service experience, communication skills to deal with challenge quickly, calmy and efficiently
· subject to meeting the above criteria, DBS and 2 satisfactory references.
Temporary cover is needed for a Monday, Thursday & Friday 13:45 – 18:45
Please note we will only review applications from candidates who can start with immediate effect as this is a temporary position. You would also be required to cover any extra holiday or sickness leave.
