Job Overview
Supporting the delivery of community-based diabetes education, this role helps individuals develop the skills and confidence to self‑manage their condition. Working alongside a multidisciplinary team, you will deliver engaging group and one‑to‑one sessions, promote healthier lifestyle choices, and improve access to education across local communities.
Main Duties
* Deliver diabetes education sessions (group and one‑to‑one) within community settings.
* Support individuals to develop self‑management skills and make informed lifestyle choices.
* Promote and deliver structured programmes such as DESMOND and other education modules.
* Manage referrals, triage patients, and coordinate education sessions.
* Monitor, record, and report on patient outcomes and programme effectiveness.
* Signpost patients to appropriate health, social, and community services.
* Work collaboratively with multidisciplinary teams and community partners.
* Contribute to service development, quality improvement, and patient‑centred care.
* Support training and awareness of diabetes education across community services.
Qualifications
Essential criteria
* Health/Social care experience NVQ Level 4.
* Microsoft 365 applications.
Desirable criteria
* Knowledge of diabetes education programs.
Experience

* Experience of meeting individuals with diabetes educational needs.
* Experience of working in a multidisciplinary team.
Desirable criteria
* Knowledge of sign posting to health care & lifestyle services.
* Experience of coordinating programmes of care or service delivery.

* Excellent organisational, time management and ability to prioritise work.
* Able to travel within geographical area to deliver education.

* Understanding behaviour change and motivational techniques.
