An exciting opportunity has arisen within the Nairn Integrated Health and Social Care Team. We are looking for a Band 4 Health and Social Care Coordinator (HSCC) to join our team on a fixed term basis for 32.10 hours per week, 5 days a week (Mon-Fri).
The HSCCs play an integral role within the Integrated Team. This involves acting as first point of access to Health and Social Care services within the district including Social Work, Care at Home, Allied Health Professionals and Community Nursing. The HSCC will work daily with the Integrated Team and providing a service to the Nairn population and other Health and Social Care Professionals. This includes gathering referral details from various sources including clients, their family members and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team to prioritise and allocate. The HSCC provides support to professional staff when dealing with urgent and critical cases in the community.
We are seeking an individual with strong communication and organisational skills, along with qualifications and/or experience in Health and Social Care.
